Instructions

-If you are not already a Hemporium member, please firstly sign up to our website (top right corner of page) and create your own account and unique password then ensure you use the same email address on the intake form below.

- Please read all questions on the medible intake form below and answer as best you are able.

​

- Once you make the payment (at the end of the intake form below) you will receive a payment submission confirmation email. 

​

- Please ensure you check your spam/junk/promotions folder for your confirmation email.

​

Once we have processed your Intake Form, you will receive a phone call to arrange an appointment with one of our Consultants. The above process usually takes no more than around 2 working days.

Cancellation Policy

Please note that once you have booked an appointment with us it means that we have reserved time with our Consultants exclusively for you. If you cancel your appointment less than 24 hours before it is scheduled to take place, you will be subject to a rebooking fee of $30. To avoid this fee, please provide cancellation notice at least 24 hours prior to your appointment.
